Movie of 2004 - LotR was shown here January 2004, so that tops my list! The Incredibles/Shaun of the Dead come in at 2nd and 3rd. This has been a depressing year, and those two somehow helped lighten it up.
Artist/Band/CD of 2004 - I'd have to say Sarah Mclachlan's Afterglow, and surprisingly, Outkast's Speakerboxx/Love below. I don't normally like hiphop, or whatever the genre they were in, but I got this for Christmas and I loved the album.
Movie Soundtrack of 2004 - The LotR soundtrack.
TV show of 2004 - The Office just started airing here this year, and my whole family's hooked.
Video/Computer Game of 2004 - Minesweeper. I'm not a big gamer.
Puppet Show of 2004 - n/a
Book of 2004 - Although I've read dozens of book this year, I'm pleased as punch that I finally was able to grab my own copy of The Hitchhiker's Guide to the Galaxy, and this is my book of the year.
